Calf implants plus fat grafting are a safe and effective way to increase the size and shape of your calves. This plastic surgery, also called calf enlargement or calf augmentation, can be performed in both men and women.

However, due to its complexity, the surgery requires a plastic surgeon with wide experience. So, you should put yourself in experienced hands and be aware of the benefits or risks of this cosmetic/reconstructive surgery.

Calf augmentation using implants or with fat transfer can be part of plastic, reconstructive and aesthetic surgery.Firstly, it is indicated for patients with physical or birth defects.

Indeed, calf implants can correct muscle imbalance secondary to congenital malformations or acquired traumatic injuries. Also, they can enhance mismatched legs in patients that suffer from muscle atrophy (reduced muscle definition) due to neurologic diseases, like poliomyelitis.

But of course, implants can also fulfill entirely aesthetic needs.

This cosmetic surgery is ideal for patients who have a genetic predisposition, and despite doing a lot of exercises, can neither increase the size of the muscles nor obtain the desired curvature.

Calf implants can greatly improve proportions with the rest of the body (mainly, the thigh area). Thus, you get a harmonious leg contour, plus an increase in the bulk and proportions of the calves.

Before Surgery

You must have a first consultation to ensure your overall suitability for calf augmentation surgery. As such, you must communicate your goals and expectations. After all, poor health and unrealistic expectations are a bad combo before surgery.

During this consultation, your calves will be measured to choose the right implant size. Also, calf implants can be custom-made to ensure you get your desired results.

At this time, your surgeon will let you know which procedure is right for you and answer any questions you may have about the surgery. He can also let you know the expected cost and financing options available to you.

He will make some preoperative recommendations too. For example, you will have to stop smoking six weeks before your surgery. Also, aspirin, anti-inflammatories, or herbal supplements must be avoided in the days prior to surgery.

You will also be required to undergo preoperative exams. They will be scheduled during your consultation.


On the day of surgery, you could receive sedation and an epidural so that you will be numbed from the waist down. However, in most cases, the surgery is performed under general anaesthetic.

During the calf implants procedure, you will be placed in the prone position (lying face down) on the operating table. This will give the surgeon better access to work during the hour required to complete this procedure.

The surgery consists of placing one or two silicone implants between the calf muscle and the skin. Semisolid, fusiform (spindle-shaped), non-rigid implants are usually used.

Sebbin calf implants are the most preferred since they have a natural feel and come in a wide range of sizes. Also, they come with a unique introducer sleeve that ensures an atraumatic insertion which has less chance of complications.

Implants are made of cohesive gel, which is denser than the one used for breast implants. The main goal is to support the strain of the calf muscles in the legs.

And don’t worry, the incision is made in the fold on the back part of the knee so that the scar won’t be noticeable. Initially, they are visible and may take a few months to lighten and completely fade.

Let’s Talk About Size and Placement

Your doctor will choose the size of the calf implants depending on the goals and expectations concerning shape. But the volume is also influenced by the stiffness of the tissues, the degree of muscle wasting, and the length of the leg.

The surgeon may need to insert one or two calf implants per leg.

When putting two implants in each leg, you can expect to gain 4-5cm more circumference. Also, in this case, your doctor most probably will insert drains and remove them 2-3 days later.

Another important subject is the placement of the implants!

Usually, calf implants are placed on top of or below the muscle. Submuscular implants give more natural results. They are completely hidden and work by pushing the existing muscle out to make it look more prominent.

However, the best position to place the calf implants will eventually be evaluated at the consultation. It will depend on the anatomical features of the legs and the desired results. So your doctor may opt to place the implants above the muscle.


Calf implants don’t give volume to the area above the ankle. In this area, fat grafting can be used to increase the diameter and achieve a more natural transition.

The fat is obtained by liposuction from other parts of the patient’s own body. It generally comes from the abdomen, thighs, hips, and double chin. So you get double the benefits! A calf augmentation and a body contouring procedure at the same time.

Calf enhancement with fat transfer is performed under general anaesthetic as a day case and takes approximately 2 to 3 hours. Although, in some cases, an overnight stay may be required.

Calf augmentation with fat transfer is a technique that prevents any possible rejection or incompatibility reactions. As it is the patient’s own fat, the final result is less harmful to the body.

However, this innovative technique isn’t for everyone. Calf augmentation surgery with fat transfer is contraindicated in thin patients who don’t have enough body fat.

Also, fat grafting is not used alone because it would give less projection than calf implants. A silicone implant is more reliable to get a more definite, defined, and noticeable increase.

For that reason, fat tissue grafts are used as a supplement to calf augmentation with implants. It allows for sculpting the zone and making it look more natural.


While all surgical procedures carry a certain degree of risk, calf implant surgery is safe and effective when performed by a board-certified plastic surgeon with experience in muscle augmentation.

Complications are rare. Infection is infrequent (less than 0.5%). Patients may notice numbness in the treated area and bruises or swelling of the legs and feet. These usually disappear in a couple of weeks.

Displacement of the implants may happen in the early post-operative period. This is prevented by following post-operative recommendations and avoiding pressure on the legs or excessive movement.

After calf augmentation surgery, it is common to spend one night in the hospital. Although, more often than not, this is not necessary (day case procedure).

You will feel mild pain and discomfort for 5 – 7 days. Patients can return to their normal daily activities after two weeks. So, you will have to take this time off work to rest.

The overall recovery period for calf augmentation surgery varies from patient to patient. It can go from 2 to 8 weeks. In the first weeks following surgery, you will have to keep your legs elevated as much as possible to speed up the healing process.

You will need help to go to the bathroom, and you will have to work your way up to slowly walking. Although, stiffness will be expected during this time.

After week two post-surgery, you can expect to begin walking normally again.

A post-surgery compression garment/bandage is fitted at the end of surgery to diminish the swelling and reduce post-operative discomfort. It will be worn permanently for a week.

Then, you will be provided with a tubular bandage to wear for the next five weeks post-surgery.

It is important not to do gymnastics or any other strenuous exercise which involves intense calf activity for the first four to six weeks after the intervention. During the recovery period, calf implants should be fixed in place and not move around!

Bear in mind that the post-operative period with two calf implants in each leg is slower than with just one. Since the space is limited, it takes the leg longer to get used to the extra volume.


Healing rates have a huge impact on your recovery and can vary widely between patients. In general, it can take six weeks or more for the final result to be noticeable. You have to let the swelling subside first.

You can expect beautiful and natural-looking results from your calf augmentation surgery. And most importantly, with implants, you get long-term, permanent results.

In the case of fat transfer to calves, the results are variable.

While fat grafting to the buttocks and breasts tends to be more consistent, less than 50% of the fat injected into this area remains after one year. Also, asymmetries are possible if one side “takes” more fat than the other.
