The objective of rhinoplasty (also called nose job or nose surgery) is to improve the appearance of the nose and at the same time maintain or improve its respiratory function.

Some small cuts are made inside the nose which can make it possible to:

  • Narrow the tip by eliminating part of the alar cartilage.
  • Reduce the ridge by straightening it.
  • Narrow the nose
  • Make the tip more turned-up, narrower or rounder.
  • Place cartilage grafts to fill certain parts of the nose.
  • Correct the deviations of the nasal septum.

Only in a few complex cases is the so-called open rhinoplasty carried out, when a small cut in made in the columnella (the bridge of skin between the lip and the tip of the nose which separates the two nostrils).

After the rhinoplasty a cast is placed on the nose to protect it and keep its shape, and some packing is placed inside the nose. It isn’t a painful operation. The day after the operation the packing will be removed and a week after the operation the cast will be removed. Inflammation usually occurs with swelling which lasts about a month. After an open rhinoplasty certain residual swelling could last longer. It should be noted that it could take various months to see the final result.

Rhinoplasty is plastic surgery which requires patience. The postoperative period is complex because the tissue doesn’t behave in a predictable way. For this reason regular postoperative visits are set up.

Normally the cast is removed after a week. It is normal that patients look strange to themselves: Not only is the nose still swollen but also the surrounding tissue is, such as the cheeks, eyes and upper lip. Furthermore, the nose feels blocked up because of the inflammation of the internal mucous.

It is after 2 weeks when the swelling starts to decrease. Normally starting from week 3 and until the 3rd to 6th months the swelling in the nose gradually reduces. The process is a little slower after open rhinoplasty.

The result of the nasal ridge can be appreciated before that of the frontal view, which takes 12 to 18 months to get a definitive result.

The deviation of the nasal septum (crooked nose) is quite a common problem that increases the complexity of the rhinoplasty. The nasal deviation can cause respiratory problems as well as aesthetic problems. It could be because of a fracture in the bones of the nose which took on a bad form during the healing process or because of a fracture in the septum.

It is necessary to do a CT (CAT) scan to see the state of the septum, to see where it is deviated and to see if there are any fractures. In these cases it is usually necessary to do an open rhinoplasty, as it is easier to see the septum and more anatomical reconstruction can be done.

The main objective of the rhinoseptoplasty (rhinoplasty of the septum to correct a crooked nose) is so that the patient can breathe better and at the same time get a good aesthetic result. Non-surgical rhinoplasty involves a technique of injecting filler which gives volume to the parts of the nose that need it. It is common to use non-surgical rhinoplasty in these cases when the nasal defect is small, like the tip of the nose, or because the person doesn’t want to have surgery to correct the nasal ridge (nasal dorsum).

Depending on the result we want to achieve and whether we want it to be more or less permanent, we usually use two kinds of nose fillers:

  • Fat tissue grafts, would be the technique which gives volume in the long term and the final shape. Although frequently one session is not enough and it would have to be repeated 4 to 6 months later.
  • Another option is to give filling with calcium hydroxyapatite, which is a biocompatible material which lasts from 12 to 18 months. It is an outpatient treatment that lasts between 30 and 45 minutes and is done with an anaesthetic cream. It is the perfect technique for patients who want to see how a definitive surgery would look like but aren’t completely sure about it or also when surgery, unless it is absolutely necessary, is not desired.

Although normally after non-surgical rhinoplasty (nose filler) normal life can be resumed the same day that you leave the clinic, some post-injection precautions should be kept in mind:

  • Preferably you should sleep on your back.
  • Avoid touching and massaging your nose.
  • Avoid, as much as possible, using glasses because resting them on the nose could move the filler downwards.

This is a recurring question that patients tend to ask. It’s logical that when a person decides to go in for rhinoplasty surgery there is a feeling of uncertainty. And here is where computer simulations come into play as they let you see a before & after result. Recent progress in photography software allows the patient to get an idea of what will be the final result that they are trying to get after the surgery.

When the surgeons use the computer simulation, it is usually so that the patient and surgeon reach an agreement on the result that they want to achieve with the surgery. But the computer simulation should in no way be considered as what the final result will be.

Undoubtedly the surgeons and the patients are the most interested in getting the desired result. But we should not forget that the plastic surgery involves working with living tissue. To change the appearance of something specific on the computer is a matter of seconds, but the computer doesn’t take into account the quality of the tissue, the thickness of the skin, how aged the skin is, and the wound healing of the tissues during the procedure nor the possible complications.

In this regard, the majority of the scientific societies don’t recommend the publication of before/after images on web pages, as there is always the question as to whether they have been manipulated. It is precisely for the adherence to these ethical principles and to avoid speculation that we don’t show photos on our website.
