BLEPHAROPLASTY SURGERY FOR A PERFECT EYELID RESHAPING

What is blepharoplasty (eyelid surgery)?

Blepharoplasty, also called eyelid surgery or eyelid lift, is one of the most common cosmetic procedures performed during the Covid-19 period.

This surgical procedure concerns the removal of excess skin and muscle from the eyelids, improving the lids flaccidity. It also reduces the excess fat and eliminates under-eye bags and dark circles.

The surgery is planned according to the needs of each patient. It can be done on the upper and lower eyelids or just one of them. Besides this, it can be performed in combination with other procedures, such as a brow lift, face-lift or skin resurfacing.

Eyelid surgery requires a plastic surgeon with wide experience. For the entire face to look harmonious and beautiful, the eyelids needs to be balanced with the rest of the face. Here is where good plastic surgeons stand out.

WHAT KIND OF PATIENTS USUALLY ASK FOR BLEPHAROPLASTY?

Blepharoplasty is one of the least invasive facial cosmetic surgeries and with better results. It rejuvenates the look in a very natural way and with no visible scars. Eyelid surgery doesn’t require hospitalization and the postoperative period is almost painless.

The surgery is designed for each individual, according to the needs and particulars of the patient. It is routinely performed on patients over 35 years old.

It is advise not to smoke, nor to take vitamin C or aspirin two weeks before the operation because this has an effect on the wound healing.

In the case of patients who cannot quit smoking completely, at least they should cut down on tobacco use.

WHAT DOES UPPER EYELID SURGERY INVOLVE?

Upper eyelid surgery is not performed just for aesthetics reasons. For some patients, saggy upper eyelid skin can obstruct the visual field. So, surgery is performed to remove this obstruction and improve vision.

To correct the upper lids, the surplus part of the skin is removed and the excess fat in the eye bags is extracted.

The scar is just inside the eyelid flap and therefore it is not visible. Normally, upper eyelid surgery is done under local anaesthetic in the same doctor’s office. Hospitalization is not necessary.

The eyelid surgery practiced by Doctor Benito is the most conservative. He doesn’t remove too much from the eye bags. With aging, we lose fat and what we could end up with is a sunken eye and more dark circles.

During recovery period, patients don’t usually have much pain, although there is discomfort during the first days after surgeryThe inflammation could stop the eyelid from closing completely and there could be bruising and swelling. There could be an accumulation of liquid in the bags under the eyes.

The stitches are removed 7 days after the surgery. From that moment on the bruising can be covered with make-up. Intense sport and the swimming pool are discouraged during the first month.

It is normal for the patient not to see immediate results. Fortunately, the eyelid heals very quickly and the inflammation disappears completely in about 4 months. The scar is practically invisible in a year.

WHAT DOES LOWER EYELID SURGERY INVOLVE?

A lower eyelid surgery is a more complicated procedure. There are various techniques available: classic, transconjunctival, Loeb and canthopexy. They are evaluated on the first visit, according to the characteristics of the patient’s eyelids, face and expectations.

Usually, for younger patients, the main issue is the underlying fat bulge. In such cases, a transconjunctival ‘scarless’ approach through the inside of the eyelid is the best option.

Lower eyelid surgery can be done under local anaesthetic, sedation, or general anaesthetic. Hospitalization is not necessary.

The skin closure can be made using either removable sutures or with glue. So, no suture removal may be required. In case of stitches, four or five days after the surgery, they are removed. Drainage massages are advised to reduce the swelling and to give elasticity to the skin.

HOW DO I REDUCE THE INFLAMMATION AND BRUISING AFTER A BLEPHAROPLASTY?

The eyelids swell easily. The most effective way to reduce inflammation is to apply cold to the zone. We recommend making some camomile tea, keeping it in the fridge and afterwards soaking gauzes in it and putting them on the eyes. You could also use cold gel masks that are sold in Chemists/Pharmacies or also frozen peas bags.

It is very important not to lie down completely flat, rather to lie down keeping the head up.

Any bruise has an evolution time from 10 to 15 days and it depends on the characteristics of each patient. You could use a venous congestion reliever. Arnica tablets and creams can also be a good preventive treatment.

After a blepharoplasty surgery, the eyes are more sensitive and if we read or watch television they can get more swollen. The recommendation is to forego reading and television during the first 4 days of rest.

After the first week, make-up can be used. However, we advise make-up that has yellow tones which are better for concealing bruises.

Some recommendations to reduce the discomfort are:

  • The first day it is better to rest, with the head raised and to use cold compresses.
  • Sleep with the head slightly raised on pillows.
  • As the eyelid doesn’t close adequately it is crucial to protect the cornea to avoid dry eyes, especially at night. For this reason, surgeon will prescribe anti-inflammatory drops, artificial tears and a protective eye ointment for night time. This ointment should be used for the first 2-3 weeks until we are sure that the eyelid shuts completely.

WHAT PERIOD OF THE YEAR IS RECOMMENDED FOR HAVING A BLEPHAROPLASTY?

The best time to have a blepharoplasty surgery is the 8 months from September to April. It is necessary to protect the scars and bruises from the damage that can be caused by the sun. All facial surgeries as facelift, nose job or blepharoplasty, leave bruises during the first 15-20 days.

If the intervention is done in summer we need to be very strict with regard to using protection from the sun. You should use a total sunblock, sunglasses and hats or wide-brimmed sun hats.

During the summer, you shouldn’t go to the beach or to the mountain where there is the risk that the scars and bruises could become pigmented. This pigmentation is visible and very difficult to treat afterwards.
